HPAC Forms
- Student Health Profession Network Form -
To be filled out by students who are interested in being assigned a HPAC advisor and in receiving communications related to the health professions. Please use Internet Explorer or Safari to access this form. If using Firefox, you will need to download the form before you can fill it out. - AAMC Competencies and Experiences Overview and Worksheet

- - Typically, due the the first Friday of December so HPAC knows you are interested in interviewing in May and you start preparing materials well in advance.
- HPAC Letter of Recommendation Form - To be filled out and given to all recommenders
- Guidance Requesting Letter of Recommendation
- - Please use this form if you are requesting an HPAC letter and it has either been a year or more since your interview, or if you requested a letter for a previous application cycle and are now reapplying. Please email an updated CV and personal statement to Ellen Grandy (egrandy@skidmore.edu). If relevant, please send any additional competency forms and transcripts as well.
- HPAC Waiver Form -
To be filled out by all students setting up a file with HPAC - - provides means to calculate GPA using AMCAS grade points. It will calculate your BCPM (Biology, Chemistry, Physics, Math) GPA along with your overall and by year.
